Document Content that Applicant should provide :
1. Domestic Health Food Re-Registration Application Form.
2. Applicant Certificate of identity, Business License or other legal registration document copy.
3. Approved Certificate Copy of Health Food (including Health Food Approved Certificate and its attachment and modifying certificate).
4. Sale certificate issued by province health food production and surveillance administration at local manufacturing site.
5. Summary of sale details in five years.
6. Summary of custom feedback about products in five years.
7. Minimum package, label and instruction sample of health food.
8. Note 1: if applicant cannot provide documents above, they should provide written reasons when applying re-registration.
9. Note 2: Documents above should be stamped by certificate owner.
